(h)confirmation that she has not been convicted of any criminal offence or been issued with a formal caution and, if she has, details of such conviction or formal caution;

[F3(i)details of the nature and scope of the applicant’s practice;

(j)the name and address of—

(i)any person, body or organisation by whom the applicant is employed, or intends to be employed as a [F4nurse, midwife or nursing associate in England],

(ii)any person, body or organisation with whom the applicant has an arrangement to provide services as a [F4nurse, midwife or nursing associate in England].]
