Regulation 20

SCHEDULE 2 E+W INFORMATION AND DOCUMENTS TO BE PROVIDED TO THE CAFCASS OR THE NATIONAL ASSEMBLY FOR WALES

1.  A certified copy of the child's birth certificate. E+W

2.  Name and address of the child's parent or guardian. E+W

3.  A chronology of the actions and decisions taken by the adoption agency with respect to the child. E+W

4.  Confirmation by the adoption agency that it has counselled, and explained to the parent or guardian the legal implications of both consent to placement under section 19 of the Act and, as the case may be, to the making of a future adoption order under section 20 of the Act and provided the parent or guardian with written information about this together with a copy of the written information provided to him. E+W

5.  Such information about the parent or guardian or other information as the adoption agency considers the officer of the Service or the Welsh family proceedings officer may need to know. E+W
