aelodaeth y person fel cadeirydd, is-gadeirydd, aelod neu gyfarwyddwr corff gwasanaeth iechyd wedi ei therfynu, heblaw oherwydd bod y swydd wedi ei dileu, oherwydd ymddiswyddiad gwirfoddol neu ad-drefnu’r corff gwasanaeth iechyd, neu oherwydd bod cyfnod y swydd y penodwyd y person amdano wedi dod i ben, neu

y person wedi ei ddiswyddo fel cadeirydd neu aelod o Fwrdd Gofal Integredig.

Os yw person wedi ei anghymhwyso o dan is-baragraff (1), bydd yr anghymhwysiad yn peidio â chael effaith pan ddaw dwy flynedd i ben gan ddechrau ar y dyddiad y terfynir cyfnod y penodiad neu unrhyw gyfnod hwy a bennwyd gan y corff a derfynodd aelodaeth y person.

Caiff Gweinidogion Cymru, pan wneir cais gan y person a anghymhwyswyd, leihau cyfnod yr anghymhwysiad a grybwyllir yn is-baragraff (2).

Pan fydd cyfnod yr anghymhwysiad o dan is-baragraff (2) yn dod i ben, ni fydd y person wedi ei anghymhwyso mwyach at ddiben yr Atodlen hon.
