3(1)Rhaid i’r Ysgrifennydd Gwladol, ar gais awdurdod tai lleol, ddarparu’r gyfryw wybodaeth ag sy’n ofynnol gan yr awdurdod—
(a)o ran a yw person yn berson y mae adran 115 o Ddeddf Mewnfudo a Lloches 1999 (eithrio rhag budd-daliadau) yn gymwys iddo, a
(b)i alluogi’r awdurdod i benderfynu a yw’r cyfryw berson yn gymwys i dderbyn cymorth o dan Bennod 2 o Ran 2.
(2)Pan roddir yr wybodaeth honno ar unrhyw ffurf ac eithrio ffurf ysgrifenedig, mae’n rhaid i’r Ysgrifennydd Gwladol gadarnhau’r wybodaeth honno yn ysgrifenedig os gwneir cais ysgrifenedig i’r Ysgrifennydd Gwladol gan yr awdurdod.
(3)Os yr ymddengys i’r Ysgrifennydd Gwladol bod unrhyw gais, penderfyniad neu newid arall mewn amgylchiadau wedi effeithio ar statws person y rhoddwyd gwybodaeth amdano yn flaenorol i awdurdod tai lleol o dan y paragraff hwn, rhaid i’r Ysgrifennydd Gwladol hysbysu’r awdurdod yn ysgrifenedig am y ffaith honno, y rheswm dros y ffaith a’r dyddiad y daeth yr wybodaeth flaenorol yn anghywir.
