remove a person from the register under section 40.
After the relevant date—

Subsection (2)(a) does not apply in relation to costs incurred before the relevant date in a case where the person is removed from the register under section 38 or 40.

in the case of a refusal or removal mentioned in subsection (1)(a) or (c), is the later of the date on which—
the period mentioned in section 41(2) expires without an appeal being made,
where such an appeal has been made, the appeal is finally determined or abandoned, and
in the case of a removal mentioned in subsection (1)(b) or (d), is the day after the day on which the person is removed from the register.
