SCHEDULE 1N.I.PERSONS BY WHOM A MEDICAL RECOMMENDATION OR MEDICAL REPORT UNDER ARTICLE 12 MAY NOT BE GIVEN
1. The applicant or a partner of, or person employed as an assistant by, the applicant.N.I.
2. A person who receives, or has an interest in the receipt of, any payments made on account of the maintenance of the patient.N.I.
3. The spouse,[F1 civil partner,] parent, father-in-law, mother-in-law, child, son-in-law, daughter-in-law, brother, brother-in-law, sister or sister-in-law of the patient.N.I.
4. A person who is living with the patient as if he or she were the spouse[F2 or civil partner] of the patient (or, if the patient is for the time being an in-patient in a hospital, was so living until the patient was admitted) and has been or had been so living for a period of not less than 6 months.N.I.
5. A person with whom the patient ordinarily resides (or, if the patient is for the time being an in-patient in a hospital, last ordinarily resided before he was admitted) and with whom he has or had been ordinarily residing for a period of not less than 5 years.N.I.
