330.—(1) Appendix 2 is amended as follows.

(2) In the heading, omit “EASA”.

(3) In the first paragraph, omit “in a Member State”.

(4) In the form—

(a) at point 3, for “State of issue” substitute “United Kingdom”;

(b) at point 8, for “Competent authority” substitute “Civil Aviation Authority (CAA)”;

(c) at point 9, for “Issuing body” substitute “CAA”;

(d) for “EASA Form 142” substitute “CAA Form 142”.

(5) After the word “Instructions:”—
