Os bydd awdurdod addysg lleol yn anfon cais ysgrifenedig at gorff llywodraethu unrhyw ysgol a gynhelir gan yr awdurdod, mae'r Rheoliadu hyn yn ei gwneud yn ofynnol i'r corff llywodraethu roi gwybodaeth y cyfeirir ati yn yr Atodlen mewn perthynas â disgyblion yn yr ysgol i'r awdurdod. Rhaid i'r corff lywodraethu roi'r gwybodaeth i'r awdurdod o fewn pedwar diwrnod ar ddeg.
Mae Rheoliad 4 yn rhagnodi yn ogystal i ba bersonau y caiff Cynulliad Cenedlaethol Cymru roi gwybodaeth am ddisgyblion unigol o dan adran 537A(4) o Ddeddf Addysg 1996.
