Cyfansoddiad timau integredig cymorth i deuluoedd
2.—(1) Rhaid i dîm gynnwys o leiaf bum person y mae pob un ohonynt yn dod o un o'r categorïau proffesiynol a ganlyn—
(a)gweithiwr cymdeithasol;
(b)nyrs; neu
(c)ymwelydd iechyd.
(2) Rhaid i dîm gynnwys o leiaf un person o bob un o'r categorïau yn is-baragraffau (a) i (c) o baragraff (1).
(3) Rhaid i un aelod o'r tîm fod yn weithiwr cymdeithasol ymgynghorol.
