ANNEX IV Specifications for the certificates of training organisations

Certificates of training organisations issued by a competent authority in accordance with this Regulation shall specify:

(a)

the competent authority issuing the certificate;

(b)

the applicant (name and address);

(c)

the type of training and/or services provided which are certified, as applicable;

(d)

a statement of the applicant’s conformity with the requirements defined in Chapter V;

(e)

the date of issue and the period of validity of the certificate.
