The information referred to in Article [F211(5)] shall be presented in the form of the entries set out in column No 1 in the following table.

For the documents referred to in [F3Article 11(4)], these entries are identified by the numbers and letters shown in columns A and B of the tables in [F4HMRC Notice 197] (column No 2 of the following table).

The order and the specific details concerning the layout of the entries shall be determined by [F6the Secretary of State, by administrative decision,] on the basis of the rules set out in Section B of this Annex. Reference number: each consignment must bear a reference number which identifies it in the consignor's accounts.
