The information referred to in Article [F211(5)] shall be presented in the form of the entries set out in column No 1 in the following table.

The order and the specific details concerning the layout of the entries shall be determined by [F6the Secretary of State, by administrative decision,] on the basis of the rules set out in Section B of this Annex.
