   Date of report.

2. 2.

   Reporting period.

3. 3.

   Name of the country.

4. 4.

   Territorial scope (in the event of a change in the territorial scope in accordance with point 3 of Section 2 of Annex VII).

5. 5.

   Information on surveillance per zone or region if more than one region is included in the territorial scope of the programme, including at least maps with data of:

   1. (a)

      Number of indicator animals tested per species;

   2. (b)

      Number of confirmed cases by species.

6. 6.

   Information on vaccination of wild animals, per zone or region if more than one region is included in the territorial scope of the programme:

   1. (a)

      Number and dates of vaccination campaigns;

   2. (b)

      Vaccine bait density achieved and, if the total size of the vaccination area per vaccination campaign is greater than 20 000 km², vaccine bait density maps;

   3. (c)

      Upon request of the Commission, electronic files containing:

      1. (i)

         Flight routes recorded during the vaccine bait distribution;

      2. (ii)

         Vaccine bait distribution (time and position of each bait released) recorded during aerial distribution if the total size of the vaccination area per vaccination campaign is greater than 20 000 km²;

   4. (d)

      Information on the monitoring of the effectiveness of the vaccination, with maps with data of:

   5. (i)

      number of animals positive to the biomarker test/number of biomarker tests performed;

   6. (ii)

      number of animals positive to serological tests/number of serological tests performed.
