SCHEDULE 2 N.I. STATEMENT OF SPECIAL EDUCATIONAL NEEDS — REGULATION 15

PART 1 N.I. introduction

1.  The following statement is made by [here insert name of Education and Library Board] in respect of the child whose name and other particulars are mentioned below. This statement is in accordance with Article 16 of the Education (Northern Ireland) Order 1996 (“the 1996 Order”) and the Education (Special Educational Needs) Regulations (Northern Ireland) 2005 (“the Regulations”). 2.  When assessing the child's special educational needs the board took into consideration the representations, evidence and advice set out in the Appendices to this statement, in accordance with regulation 10 of the Regulations. N.I.

PART 2 N.I. special educational needs

[Here set out the child's special educational needs, in terms of the child's learning difficulties which call for special educational provision, as assessed by the board.]

PART 3 N.I. special educational provision other than placement

Objectives

[Here specify the objectives which the special educational provision for the child should aim to meet.]

Educational provision to meet needs and objectives

[Here specify the special educational provision which the board considers appropriate to meet the needs specified in Part 2 and to meet the needs specified in this Part, and in particular specify—

(a) any appropriate facilities and equipment, staffing arrangements and curriculum;

(b) any appropriate modifications to the application of the Northern Ireland curriculum;

(c) any appropriate exclusions from the application of the Northern Ireland curriculum, in detail, and the provision which it is proposed to substitute for any such exclusions in order to maintain a balanced and broadly based curriculum; and

(d) where residential accommodation is appropriate, that fact.]

Monitoring

(a) regularly monitoring progress in meeting the objectives specified in this Part;

(b) establishing targets in furtherance of these objectives;

(e) regularly monitoring the appropriateness of any provision substituted for exclusions from the application of the Northern Ireland curriculum.

PART 4 N.I. special educational provision: placement

(b) the provision for his education otherwise than at a school which the board considers appropriate.
