9Cynnal asesiadau iechyd meddwl sylfaenol

(1)Mae asesiad iechyd meddwl sylfaenol yn ddadansoddiad o iechyd meddwl unigolyn sy'n dynodi–

(a)y driniaeth iechyd meddwl sylfaenol leol (os oes un) a allai wella iechyd meddwl yr unigolyn neu atal dirywiad ynddo (rhaid darparu unrhyw driniaeth a ddynodir felly: gweler adrannau 3 a 5); a

(b)y gwasanaethau eraill (os oes rhai) a allai wella iechyd meddwl yr unigolyn neu atal dirywiad ynddo.

(2)Rhaid i'r partneriaid iechyd meddwl lleol sicrhau bod yr asesiad yn cael ei gynnal gan unigolyn sy'n gymwys i gynnal asesiadau iechyd meddwl sylfaenol o dan reoliadau a wneir o dan adran 47.

(3)Mae'r cyfeiriad at wasanaethau eraill yn is-adran (1)(b) yn gyfeiriad at–

(a)gwasanaethau iechyd meddwl eilaidd;

(b)gwasanaethau o fath a ddarperir fel arfer gan ddarparwyr gofal sylfaenol;

(c)gwasanaethau gofal cymunedol (nad ydynt yn wasanaethau iechyd meddwl eilaidd);

(d)gwasanaethau a ddarperir o dan Ran III o Ddeddf Plant 1989 (nad ydynt yn wasanaethau iechyd meddwl eilaidd);

(e)gwasanaethau tai neu wasanaethau llesiant; ac

(f)addysg neu hyfforddiant a all fod o fudd i iechyd meddwl unigolyn.
