An application for the grant of a licence under this Act shall be made in such form, and include such declarations and information, as the Department may require.

The Department may require an applicant to furnish such further information as it may consider necessary for dealing with the application.

If a person fails without reasonable excuse to furnish information when required to do so under subsection (2), the Department may decline to proceed further with the application and refuse to grant the licence.

The provisions of this Act apply to the renewal of a licence as they apply to the grant of a licence.
