Notice of application for an order under section 31(1)(i) of the Act for the consent of the court to such alterations as are referred to in section 31(1)(a) to (d) thereof shall be in Form 209.

Notice of intention to object under paragraph 5 of Schedule 8 to the Act shall be in Form 210.

An order consenting to such alterations shall be in Form 211.
