1. These Regulations may be cited as the Gaming Machine (Form of Amusement Permit) Regulations (Northern Ireland) 2004 and shall come into operation on 30th September 2004.
2.—(1) An amusement permit shall be granted in Form 1 and renewed in Form 2 in the Schedule.
(2) There shall be entered on each permit such of the entries as are therein set out as relate to the grant or renewal of the permit by the district council.
3. The Gaming Machine (Form of Amusement Permit) Regulations (Northern Ireland) 1986(3) are hereby revoked.

These Regulations prescribe the form of amusement permits.
They make provision for the form of an amusement permit granted for the purposes of Article 108(1)(c) or 108(1)(ca) of the Betting, Gaming, Lotteries and Amusements (Northern Ireland) Order 1985. The latter permit is subject to conditions restricting access to the premises for which it is granted, or to an area within those premises, to people aged 18 or over.
They revoke and re-enact with changes the Gaming Machine (Form of Amusement Permit) Regulations (Northern Ireland) 1986.
