13.  Where the teacher holds a degree or equivalent qualification, other than by successfully completing a course for the initial teacher training of teachers –

(a) its title;

(b) the class of the degree or qualification;

(c) its subject;

(d) the date of its award; and

(e) the name of the institution by which it was awarded.
