SCHEDULE 2PARTICULARS TO BE INCLUDED IN THE WRITTEN NOTICE SENT TO THE EXECUTIVE BY THE DIVING CONTRACTOR
The particulars referred to in regulation 7(1) are –

Name, address and telephone number of the diving contractor.

Legal status of the diving contractor (for example: limited company, partnership, sole proprietor).

Name, address and telephone number (where different from 1) of the person to be contacted, in case of need, by the Executive.
4
The signature and full name of the person supplying the particulars.
5
The date upon which the particulars are supplied.
