A person moving poultry, other captive birds or their products under a licence granted under these Regulations must, as soon after the movement as is reasonably practicable, make a record of—
a
what is moved, including its quantity;
b
the date of the movement;
c
the name of the consignor;
d
the address of the premises from which the movement started;
e
the registration number of any vehicle used;
f
the name of the consignee; and
g
the address of the destination.

The person making such a record must retain it for at least twelve months from the date the movement was completed.
