Short death certificates37

Subject to Article 40A of the 1976 Order, the Registrar General, or any registrar, may as regards any entry in a register of deaths, issue a short death certificate.

The form of a short death certificate shall be Form 20 F1, 20A or 20B.

The particulars to be furnished by an applicant for a short death certificate shall be the following particulars relating to the deceased person to whom the application relates—

a

the name and surname of the person,

b

the date and place of death of the person, and

c

the usual address of the person prior to the death.

The furnishing of any of the particulars listed in paragraph (3) may be dispensed with in any case where it is not reasonably practicable for the applicant to furnish such particulars.
