An agreement must include terms which have the same effect as paragraph 1 of schedule 3A.

An agreement which includes the provision of any vaccination services must—
a
in relation to all such services, contain a term which has the same effect as that specified in paragraph 2 of schedule 3A; and
b
in relation to each such service which the provider is required to provide, contain terms which have the same effect as those specified in schedule 3A, which are relevant to that service.
