Proposed monitor's statement and consent to act

(1)
A statement under section A6(1)(b) of the Insolvency Act 1986 must be headed “Proposed monitor's statement and consent to act” and must contain the following—
(a)
a certificate that the proposed monitor is qualified to act as an insolvency practitioner in relation to the company,
(b)
the proposed monitor's IP number,
(c)
the name of the relevant recognised professional body which is the source of the proposed monitor's authorisation to act in relation to the company, and
(d)
a statement that the proposed monitor consents to act as monitor in relation to the company.
(2)
In this paragraph “IP number” means the number assigned to an office-holder as an insolvency practitioner by the Secretary of State.
