The Registrar General for Scotland, in exercise of the powers conferred on him by paragraph 1(1) of Schedule 1 to the Adoption (Scotland) Act 1978(1) and with the approval of the Secretary of State, hereby makes the following Regulations:
1. These Regulations may be cited as the Adopted Children Register (Form of Entry) (Scotland) Regulations 1991 and shall come into force on 1st July 1991.
2. The entry to be made in the Adopted Children Register pursuant to a direction contained in an adoption order shall be in the form set out in the Schedule to these Regulations.
3. The Registration of Births, Deaths and Marriages (Adopted Children Register) (Scotland) Regulations 1975(2) are hereby revoked.

These Regulations specify the form of entry to be made in the Adopted Children Register maintained by the Registrar General for Scotland following a direction in an adoption order. The new form omits reference to the NHS number of the adopted child but otherwise it is the same as the form applicable prior to the coming into force of these Regulations.
The Regulations also revoke the Registration of Births, Deaths and Marriages (Adopted Children Register) (Scotland) Regulations 1975 (S.I. 1975/2091) which previously specified the form of entry.
