Where that part of the registration document which relates to a change of keeper has been, or may have been, lost, stolen or destroyed, notification in accordance with paragraph (3) shall be effected by an application to the Secretary of State for the issue of a new registration document accompanied by a fee of £19.

3B

On receiving notification in accordance with paragraphs (3) or (3A), the Secretary of State shall, subject to regulation 15—

a

record the change in the register, and

b

issue to the new registered keeper a new registration document.
