Regulation 20 and paragraph 35 of Schedule 3
A patient leaflet shall include—
1. The name of the party or parties comprising the contractor.E+W
2. In the case of an agreement with [F1a dental corporation]—E+W
(a)the names of the directors, chief executive and secretary of [F2that corporation], in so far as those positions exist in relation to [F3the dental corporation]; and
(b)the address of [F4the corporation's] registered office.
