In Schedule 3 for Form A1, Form B1, Form G, Form H and Form J substitute the Form A1, Form B1, Form G, Form H and Form J as set out in the Schedule to these Regulations.
(2)
In Schedule 3 for Form K substitute the Form K as set out in the Schedule to these Regulations.
