A form of proxy sent out with the notice of a meeting must not have a person’s name or description inserted in it.

The only form of proxy that may be used at a meeting is one sent with the notice summoning the meeting.

A form of proxy must be signed by the principal, or by a person authorised by the principal to do so (either generally or with reference to a particular meeting).

If a form of proxy is signed by a person other than the principal, the nature of that person’s authority must be stated.

A form of proxy is invalid if it does not comply with any part of this rule.
