Regulation 4(a)

SCHEDULE 1Particulars of a death to be furnished by a registrar of births and deaths in England and Wales

1.  The surname and forenames of the deceased.

2.  The date of death.

3.  The sex of the deceased.

4.  The date of birth of the deceased.

5.  The usual address of the deceased at the date of death.

6.  The date on which the death was registered.

7.  Any identifying reference number.

8.  The district where the death was registered or, in the case of a declaration, the district where the death is required to be registered.

Regulation 4(b)

SCHEDULE 2Particulars of a death to be furnished by the Registrar General for England and Wales

1.  The surname and forenames of the deceased.

2.  The date of death.

3.  The sex of the deceased.

4.  The maiden surname of a deceased married, widowed or divorced woman.

5.  The date of birth of the deceased.

6.  The usual address of the deceased at the date of death.

Regulation 4(c)

SCHEDULE 3Particulars of a death to be furnished by the Registrar General of Births, Deaths and Marriages for Scotland as soon as reasonably practicable

1.  The surname and forenames of the deceased.

2.  The date of death.

3.  The sex of the deceased.

4.  The date of birth of the deceased.

5.  The usual address of the deceased at the date of death.

6.  The date on which the death was registered.

7.  Any identifying reference number.

8.  The district where the death was registered.

Regulation 4(d)

SCHEDULE 4Particulars of a death to be furnished by the Registrar General of Births, Deaths and Marriages for Scotland as the Secretary of State may direct

1.  The surname and forenames of the deceased.

2.  The date of death.

3.  The sex of the deceased.

4.  The maiden surname of a deceased married, widowed or divorced woman.

5.  The date of birth of the deceased.

6.  The place of usual residence of the deceased.

7.  The district where the death was registered.
