Where an application for planning permission is made, the applicant must certify, in a form published by the Secretary of State or in a form substantially to the same effect, that the relevant requirements of article 13 have been satisfied.

If an applicant has cause to rely on article 13(6), the certificate must state the relevant circumstances.
