[F1 6.   Where the passenger is a person required by regulation 4(2) to self-isolate, and intends to undertake a test in accordance with Schedule 2A— E+W

(a) the name of the test provider,

(b) the test reference number provided to them by the test provider in accordance with paragraph 4(d) of that Schedule.]
