Materion ffurfiol deiseb
10.—(1) Rhaid i ddeiseb ddatgan ar bob dalen—
(a)enw'r awdurdod y mae'n cael ei chyfeirio ato; a
(b)y newid cyfansoddiadol y ceisir y refferendwm mewn perthynas ag ef.
(2) Rhaid i ddeiseb gynnwys, ar bob dalen, ddatganiad yn y termau a nodir yn Atodlen 1 i'r Rheoliadau hyn neu mewn termau i'r un perwyl.
(3) Mewn perthynas â phob person sy'n llofnodi deiseb, rhaid i'r wybodaeth ganlynol gael ei rhoi—
(a)enw cyntaf a chyfenw a chyfeiriad y person hwnnw; a
(b)y dyddiad y mae'r person hwnnw'n llofnodi'r ddeiseb.
(4) Rhaid i ddeiseb gynnwys, datganiad sy'n cynnwys enw a chyfeiriad llawn y person (y cyfeirir ato fel “trefnydd y ddeiseb” yn y Rhan hon) y mae gohebiaeth sy'n ymwneud â'r ddeiseb i gael ei hanfon ato, neu rhaid i ddatganiad o'r fath gyd-fynd â'r ddeiseb.
(5) Os caiff deisebau eu cyfuno cyn eu cyflwyno i'r awdurdod—
(a)rhaid i drefnydd deiseb pob un o'r deisebau cyfansoddol benderfynu ar enw'r person (p'un a yw'r person hwnnw'n drefnydd deiseb unrhyw un o'r deisebau cyfansoddol neu beidio) sydd i fod yn drefnydd deiseb at ddibenion y ddeiseb gyfun; a
(b)rhaid i drefnydd deiseb y ddeiseb gyfun hysbysu'r awdurdod o enw a chyfeiriad llawn trefnydd y ddeiseb.
