Adolygu penderfyniadau – Cam 1
5.—(1) Caiff person y mae penderfyniad a grybwyllir yn rheoliad 4 uchod yn cael ei gyfeirio ato, wneud cais i'r Cynulliad Cenedlaethol ymhen heb fod yn hwyrach na 30 diwrnod yn dilyn dyddiad y llythyr yn rhoi gwybod i'r person hwnnw am y penderfyniad i'w adolygu, am adolygiad o'r penderfyniad hwnnw.
(2) Rhaid i gais am adolygiad fod yn ysgrifenedig ar ffurf a bennwyd gan y Cynulliad Cenedlaethol a rhaid iddo nodi —
(a)enw a chyfeiriad y ceisydd, ac ar ba sail y ceisir yr adolygiad;
(b)y cynllun cymhorthdal y ceisir yr adolygiad mewn perthynas ag ef a'r flwyddyn IACS y cyfeiriodd y penderfyniad ati;
(c)y penderfyniad sydd i'w adolygu a'i ddyddiad;
(ch)manylion llawn y seiliau y ceisir yr adolygiad arnynt; a
(d)y newid yn y penderfyniad a geisir.
(3) Mae cais o dan y rheoliad hwn i'w drin fel un sydd wedi'i wneud os daw i law'r Cynulliad Cenedlaethol yn y Swyddfa Ranbarthol y gwnaed y penderfyniad gwreiddiol ynddi.
