Hysbysu marwolaeth, salwch a digwyddiadau eraill

38.—(1) Rhaid i'r person cofrestredig roi hysbysiad i swyddfa briodol y Cynulliad Cenedlaethol yn ddi-oed os digwydd—

(a)marwolaeth unrhyw ddefnyddiwr gwasanaeth, gan nodi amgylchiadau'r farwolaeth;

(b)brigiad unrhyw glefyd heintus yn y cartref gofal sydd ym marn unrhyw ymarferydd meddygol cofrestredig sy'n gofalu am bersonau yn y cartref gofal yn ddigon difrifol i gael ei hysbysu felly;

(c)unrhyw anaf difrifol i ddefnyddiwr gwasanaeth;

(ch)salwch difrifol defnyddiwr gwasanaeth mewn cartref gofal nad oes nyrsio'n cael ei ddarparu ynddo;

(d)unrhyw ddigwyddiad yn y cartref gofal sy'n effeithio ar les neu ddiogelwch unrhyw ddefnyddiwr gwasanaeth;

(dd)unrhyw ladrad, bwrgleriaeth neu ddamwain ddifrifol yn y cartref gofal;

(e)unrhyw honiad o gamymddwyn gan y person cofrestredig neu unrhyw berson sy'n gweithio yn y cartref gofal.

(2) Rhaid i unrhyw hysbysiad a roddir yn unol â'r rheoliad hwn ar lafar gael ei gadarnhau yn ysgrifenedig.
