1. Enw a chyfeiriad busnes pob person cofrestredig.
2. Cymwysterau a phrofiad perthnasol pob person cofrestredig.
3. Nifer, cymwysterau a phrofiad perthnasol y staff sy'n gweithio yn y cartref gofal.
4. Strwythur trefniadol y cartref gofal.
5. Ystod oedran a rhyw y defnyddwyr gwasanaeth y bwriedir darparu llety ar eu cyfer.
6. Ystod yr anghenion y bwriedir i'r cartref gofal eu diwallu.
7. A gaiff gwasanaeth nyrsio ei ddarparu.
8. Unrhyw feini prawf a ddefnyddir ar gyfer derbyniadau i'r cartref gofal, gan gynnwys polisi a gweithdrefnau (os oes rhai) y cartref gofal ar gyfer derbyniadau brys.
