ATODLEN 6MATERION SYDD I'W MONITRO MEWN CARTREF GOFAL Y MAE PLANT YN CAEL EU LLETYA YNDDO
1. Cydymffurfedd ag unrhyw gynllun ar gyfer gofal y plentyn a baratowyd gan yr awdurdod lleoli a chynllun lleoliad pob plentyn sy'n cael ei letya yn y cartref gofal.
2. Adneuo a rhoi arian a phethau gwerthfawr eraill a drosglwyddyd er mwyn eu cadw'n ddiogel.
3. Bwydlenni dyddiol.
4. Pob damwain a niwed sy'n cael eu dioddef yn y cartref neu gan y plant sy'n cael eu lletya yno.
5. Unrhyw salwch y mae'r plant sy'n cael eu lletya yn y cartref yn ei gael.
6. Cwynion mewn perthynas â'r plant sy'n cael eu lletya yn y cartref gofal a'u canlyniadau.
7. Unrhyw honiadau neu amheuon o gamdriniaeth mewn perthynas â'r plant sy'n cael eu lletya yn y cartref gofal a chanlyniad unrhyw ymchwiliad.
8. Cofnodion recriwtio staff a chofnodion ynghylch cynnal y gwiriadau angenrheidiol ar gyfer gweithwyr newydd yn y cartref gofal.
9. Ymwelwyr â'r cartref gofal ac â'r plant yn y cartref gofal.
10. Hysbysiadau o'r digwyddiadau a restrir yn Atodlen 5 i Reoliadau Cartrefi Plant (Cymru) 2002.
11. Unrhyw absenoldeb diawdurdod o'r cartref gofal gan blentyn sy'n cael ei letya yno.
12. Defnyddio unrhyw fesurau disgyblu mewn perthynas â'r plant sy'n cael eu lletya yn y cartref gofal.
13. Defnyddio ataliadau corfforol mewn perthynas â'r plant sy'n cael eu lletya yn y cartref gofal.
