Cofnodion ynglyn â gwasanaethau
18.—(1) Rhaid i'r person cofrestredig gadw'r cofnodion canlynol a'u cadw'n gyfoes, gan ddangos ar gyfer pob person y mae'r asiantaeth yn darparu gwasanaethau cymorth mabwysiadu iddo —
(a)enw llawn;
(b)dyddiad geni;
(c)a yw'r person:
(i)yn blentyn y caniateir ei fabwysiadu, yn rhiant neu'n warcheidwad iddo;
(ii)yn berson sy'n dymuno mabwysiadu plentyn;
(iii)yn berson sydd wedi'i fabwysiadu, ei riant, rhiant naturiol, cyn warcheidwad neu'n berson perthynol;
(ch)disgrifiad o'r gwasanaethau y gofynnwyd amdanynt;
(d)disgrifiad o'r anghenion fel y'u haseswyd gan awdurdod lleol;
(dd)disgrifiad o'r gwasanaethau a ddarparwyd;
(e)a yw'r gwasanaethau yn cael eu darparu ar ran awdurdod lleol o dan reoliadau a wnaed o dan adran 3(4)(b) o Ddeddf 2002.
(2) Rhaid i'r cofnodion a bennir ym mharagraff (1) gael eu cadw am o leiaf bymtheg a thrigain o flynyddoedd o ddyddiad y cofnod diwethaf.
