Cofnodion y mae'n rhaid i gludwyr eu cadw
5. Mae'n rhaid i unrhyw gludwr sy'n casglu llaeth neu gynhyrchion llaeth ar ran prynwr gadw cofnodion, a'u dal am y cyfnod perthnasol, yn cynnwys manylion yr holl feintiau o laeth a chynhyrchion llaeth a gesglir felly, yn nodi —
(a)yr amser a'r dyddiad y'u casglwyd gan bob cynhyrchwr;
(b)yr amser a'r dyddiad y samplwyd llaeth neu gynhyrchion llaeth pob cynhyrchwr;
(c)manylion y cynhyrchwr dan sylw;
(ch)maint y llaeth a gasglwyd (gan gynnwys copi o dderbynneb y tancer mewn achosion y cyfeirir atynt ym mharagraffau 2(2)(c) a 3(e));
(d)manylion y cynhyrchwr dan sylw,
(dd)maint y llaeth a ddanfonwyd, ac enw a chyfeiriad pob safle derbyn;
(e)ffynonellau'r holl laeth a gludir ar bob tancer; a
(f)manylion unrhyw achos o gamweithio o ran unrhyw offer a ddefnyddir ganddo neu ganddi.
