Cais i ddiddymu cofrestriad
12.—(1) Yn y rheoliad hwn —
ystyr “cais i ddiddymu” yw cais gan y person cofrestredig o dan adran 15(1)(b) o'r Ddeddf i ddiddymu ei gofrestriad;
ystyr “dyddiad effeithiol arfaethedig” yw'r dyddiad y mae'r person cofrestredig yn gofyn amdano fel y dyddiad pan fydd y diddymiad y gwneir y cais amdano i gael effaith.
(2) Rhaid i gais am ddiddymu —
(a)cael ei wneud yn ysgrifenedig;
(b)cael ei anfon neu ei draddodi i swyddfa briodol yr awdurdod cofrestru dim llai na 3 mis cyn y dyddiad effeithiol arfaethedig neu gyfnod llai cyn y dyddiad hwnnw y gellir cytuno arno gyda'r awdurdod cofrestru;
(c)pennu'r dyddiad effeithiol arfaethedig.
