11. Mewn perthynas ag unrhyw berson, ac eithrio'r ceisydd, sy'n gofalu neu sydd i ofalu am blant perthnasol—
(a)pa un a yw'r person yn preswylio, neu a fwriedir iddo breswylio yn y fangre ai peidio;
(b)os yw'r person yn berthynas i'r ceisydd, natur y berthynas honno;
(c)pa un a yw'r person yn gweithio, neu a fwriedir iddo weithio, ar sail amser llawn ynteu ar sail ran-amser, ac os ar sail ran-amser, y nifer o oriau bob wythnos y bwriedir i'r person hwnnw weithio;
(ch)y dyddiad y dechreuodd y person weithio, neu y bwriedir iddo ddechrau gweithio;
(d)gwybodaeth ynghylch cymwysterau, profiad a sgiliau'r person hwnnw, i'r graddau y mae'n berthnasol i'r gwaith sydd i'w gyflawni gan y person hwnnw;
(dd)datganiad gan y ceisydd i'r perwyl ei fod wedi ei fodloni ynghylch dilysrwydd y cymwysterau ac wedi gwirio'r profiad a'r sgiliau y cyfeirir atynt yn is-baragraff (d);
(e)datganiad gan y ceisydd ynghylch—
(i)addasrwydd cymwysterau'r person ar gyfer y gwaith y bwriedir i'r person ei gyflawni,
(ii)pa un a oes gan y person y sgiliau angenrheidiol ai peidio ar gyfer gwaith o'r fath, a
(iii)addasrwydd y person i weithio gyda phlant [F1o dan ddeuddeng mlwydd oed] a bod mewn cysylltiad â phlant o'r fath yn rheolaidd;
(f)datganiad gan y person ynglŷn â chyflwr ei iechyd corfforol a meddyliol;
(ff)datganiad gan y ceisydd i'r perwyl bod y person yn ffit yn gorfforol ac yn feddyliol ar gyfer y gwaith y bwriedir i'r person hwnnw ei gyflawni;
(g)datganiad gan y ceisydd i gadarnhau pa un a fodlonwyd ef ai peidio ynghylch dilysrwydd hunaniaeth y person, ac yn nodi'r dull a ddefnyddiodd y ceisydd i fodloni ei hunan ynglŷn â hynny, a pha un a yw'r ceisydd wedi cael copi o dystysgrif geni'r person ai peidio;
(ng)cadarnhad gan y ceisydd bod ganddo ffotograff diweddar o'r person;
(h)datganiad gan y ceisydd i'r perwyl ei fod wedi cael—
(i)dau eirda mewn perthynas â'r person, a bod y ceisydd wedi ei fodloni ynghylch dilysrwydd y geirdaon hynny,
(ii)hanes cyflogaeth llawn, ynghyd ag esboniad boddhaol o unrhyw fylchau yng nghyflogaeth y person, a
(iii)os oedd cyflogaeth neu swydd flaenorol y person yn ymwneud â phlant, cadarnhad, i'r graddau y bo'n rhesymol ymarferol, o'r rheswm pam y daeth y gyflogaeth neu'r swydd honno i ben
