Ffurf y cais ac amseriad y cyfweliad
4.—(1) Pan fo'r ddyletswydd yn rheoliad 3 yn gymwys rhaid i'r pennaeth roi hysbysiad ysgrifenedig i'r rhiant o'r materion canlynol—
(a)dyddiad, amser a hyd y cyfweliad ailintegreiddio,
(b)diben y cyfweliad, ac
(c)dyletswydd y llys, wrth benderfynu pa un ai i wneud gorchymyn rhianta ai peidio mewn perthynas â rhiant o dan adran 20 o Ddeddf Ymddygiad Gwrthgymdeithasol 2003(1), i roi ystyriaeth i fethiant gan y rhiant heb esgus rhesymol i fynychu cyfweliad ailintegreiddio pan gaiff gais i wneud hynny yn unol â'r Rheoliadau hyn.
(2) Yn ddarostyngedig i baragraff (4), rhaid i'r cyfweliad gael ei gynnal ar ddiwrnod ysgol o fewn y cyfnod—
(a)sy'n dechrau ar y diwrnod cyntaf y mae'r gwaharddiad yn ymwneud ag ef, a
(b)sy'n gorffen ar y pymthegfed diwrnod ysgol sy'n dod ar ôl y diwrnod olaf y mae'r gwaharddiad yn ymwneud ag ef (pa un ai a yw'r diwrnod ysgol hwnnw yn dod o fewn yr un tymor ai peidio).
(3) Cyn rhoi'r hysbysiad rhaid i'r pennaeth wneud ymdrech resymol i drefnu'r cyfweliad ar ddyddiad ac ar amser (o fewn y cyfnod) a awgrymir gan y rhiant.
(4) Caniateir cynnal y cyfweliad ar ddiwrnod o fewn y cyfnod y cyfeirir ato ym mharagraff (2) nad yw'n ddiwrnod ysgol os bydd y pennaeth a'r rhiant yn cytuno ar hynny.
(5) Rhaid i'r hysbysiad gael ei roi dim hwyrach na chwe niwrnod ysgol cyn dyddiad y cyfweliad ailintegreiddio.
