5.—(1) Rhaid i'r meddiannydd, cyn gynted ag y bo'n rhesymol ymarferol ar ôl cymryd sampl, gofnodi—
(a)y math o sampl a gymerwyd;
(b)y dyddiad pryd y cymerwyd y sampl;
(c)pan fo mwy nag un haid ar y daliad, dull adnabod yr haid y cymerwyd y sampl ohoni;
(ch)oed yr haid a samplwyd;
(d)y labordy yr anfonwyd y sampl iddo;
(dd)dyddiad cigydda arfaethedig yr haid a samplwyd.
(2) Rhaid i'r meddiannydd gofnodi canlyniad pob prawf pan gaiff ef gan y labordy.
