Pob disgybl
7.—(1) Manylion byr am gyflawniadau'r disgybl mewn unrhyw faes dysgu, unrhyw bwnc neu weithgaredd, gan gynnwys Tystysgrif Graidd Cymhwyster Bagloriaeth Cymru, a'r rheini'n gyflawniadau nas crybwyllir yn unman arall yn yr Atodlen hon sy'n rhan o'r cwricwlwm ysgol ac am sgiliau a galluoedd y disgybl ac ynghylch cynnydd cyffredinol y disgybl yn yr ysgol yn ystod y cyfnod y mae'r wybodaeth yn ymwneud ag ef.
(2) Manylion y trefniadau y caniateir trafod yr adroddiad odanynt gydag athrawon y disgybl gan riant y disgybl neu, yn achos disgybl sy'n 18 oed neu drosodd, gan y disgybl.
(3) Manylion unrhyw gymhwyster perthnasol a gymeradwywyd neu unrhyw uned neu gredyd tuag at gymhwyster o'r fath a gafodd y disgybl yn ystod y cyfnod y mae'r wybodaeth yn ymwneud â hi ac na chyfeiriwyd ato yn rhywle arall yn yr Atodlen hon.
(4) Crynodeb o gofnod presenoldeb y disgybl yn ystod y cyfnod y mae'r wybodaeth yn ymwneud ag ef yn dangos nifer yr absenoldebau awdurdodedig ac anawdurdodedig (o fewn ystyr Rheoliadau Addysg (Cofrestru Disgyblion) (Cymru) 2010(1)) a nifer y presenoldebau posibl.
