(1)At ddibenion gwella effeithiolrwydd gwasanaethau iechyd meddwl a ddarperir i glaf perthnasol, rhaid i ddarparydd gwasanaeth iechyd meddwl gymryd pob cam rhesymol i sicrhau–
(a)bod gwahanol wasanaethau iechyd meddwl y mae'n gyfrifol am eu darparu i glaf yn cael eu cydgysylltu â'i gilydd; a
(b)bod y gwasanaethau iechyd meddwl y mae'n gyfrifol am eu darparu i'r claf yn cael eu cydgysylltu ag unrhyw wasanaethau eraill y mae unrhyw ddarparydd gwasanaeth iechyd meddwl arall yn gyfrifol am eu darparu;
(c)bod y gwasanaethau iechyd meddwl y mae'n gyfrifol am eu darparu yn cael eu cydgysylltu ag unrhyw wasanaethau sy'n ymwneud ag iechyd meddwl ac sy'n cael eu darparu ar gyfer y claf gan gorff gwirfoddol.
(2)Caiff darparydd gwasanaeth iechyd meddwl geisio cyngor cydgysylltydd gofal claf o ran sut y dylai'r darparydd gyflawni ei ddyletswydd o dan is-adran (1).
(3)Caiff cydgysylltydd gofal ar unrhyw adeg roi cyngor i ddarparydd gwasanaeth iechyd meddwl o ran sut y dylai'r darparydd gyflawni ei ddyletswydd o dan is-adran (1).
(4)Rhaid i ddarparydd gwasanaeth iechyd meddwl roi sylw i unrhyw gyngor a roddir o dan is-adran (2) neu (3) wrth iddo gyflawni ei ddyletswydd o dan is-adran (1).
(5)Yn yr adran hon gwasanaethau iechyd meddwl yw–
(a)gwasanaethau iechyd meddwl eilaidd;
(b)gwasanaethau o dan Ran 1 o'r Mesur hwn;
(c)pethau a wneir wrth arfer pwerau awdurdod lleol yn adran 8 o Ddeddf Iechyd Meddwl 1983 mewn cysylltiad â pherson sy'n destun gwarcheidiaeth yr awdurdod.
(6)Yn yr adran hon ystyr “corff gwirfoddol” yw corff y mae ei weithgareddau'n cael eu cyflawni mewn modd ac eithrio ar gyfer gwneud elw.
