3. The form set out in Schedule 2 to these Regulations, or a form to the like effect, is the prescribed form for the purposes of an application under sections 112 (criminal conviction certificates)(1), 113A (criminal record certificates)(2), 113B (enhanced criminal record certificates)(3), 114 (criminal record certificates: Crown employment)(4) and 116 (enhanced criminal record certificates: judicial appointments and Crown employment)(5).
