Modifications etc. (not altering text)
1. The names and addresses of the responsible paying authority for the purpose of sub-paragraph (a) of paragraph (1) of regulation (7) and the insured person, and the period of the insured person's service for the purpose of the provisions of Part II of the Act of 1959 relating to the making of a payment in lieu of contributions at the end of a person's period of service in a non-participating employment.U.K.
2. The name or such other particulars as will sufficiently identify the recognised superannuation scheme, and the retirement benefits by way of pension payable under that scheme in relation to that period of service, or such part of them as has to be taken into account to constitute them equivalent pension benefits.U.K.
3. F2... The date as from which such benefits are payable, F2... and the person to whom they are payable, and, where that person is not the insured person, a statement that that person is a trustee of the benefits so payable for the insured person.U.K.

4. A statement in such form as the Minister shall approve that the insured person is absolutely and indefeasibly entitled to the benefits to which the certificate relates under or by virtue of the recognised superannuation scheme (or would be so entitled but for their being capable of being terminated or suspended for any cause prescribed for the purpose of paragraph (c) of subsection (1) of section 8 of the Act of 1959), and that those benefits are equivalent pension benefits for the purpose of Part II of the Act of 1959 in relation to the period of the insured person's service, and that they are secured in such manner and such provision is made for their payment as to make the insured person assured of them for the purpose of the provisions of the said Part II relating to the making of a payment in lieu of contributions at the end of a person's period of service in a non-participating employment.U.K.
