Arweiniad i'r cynllun lleoli oedolion
5.—(1) Rhaid i'r person cofrestredig lunio arweiniad ysgrifenedig i'r cynllun lleoli oedolion (y cyfeirir ato yn y Rheoliadau hyn fel “arweiniad i'r cynllun lleoli oedolion”) ac mae'n rhaid iddo gynnwys —
(a)crynodeb o'r datganiad o ddiben;
(b)naill ai crynodeb o'r adroddiad archwilio diweddaraf neu gopi o'r adroddiad hwnnw;
(c)crynodeb o'r drefn gwyno a baratowyd o dan reoliad 21; ac
(ch)cyfeiriad a Rhif ffôn swyddfa briodol y Cynulliad Cenedlaethol.
(2) Rhaid i'r person cofrestredig —
(a)darparu copi o'r arweiniad cyntaf i'r cynllun lleoli oedolion i swyddfa briodol y Cynulliad Cenedlaethol;
(b)darparu copi o'r arweiniad cyfredol i bob oedolyn pan gaiff ei leoli gyntaf o dan y cynllun lleoli oedolion;
(c)darparu ar gais gopi o'r arweiniad cyfredol i unrhyw oedolyn y mae caniatâd i'w leoli o dan y cynllun neu i gynrychiolydd yr oedolyn.
(3) Rhaid llunio'r arweiniad i'r cynllun lleoli oedolion mewn fformat sy'n briodol i anghenion oedolion perthnasol.
