Yr wybodaeth ychwanegol y mae'n rhaid trefnu iddi fod ar gael
4.—(1) Yn ddarostyngedig i baragraff (3), yn achos disgybl cofrestredig sydd mewn ysgol a gynhelir ac mewn unrhyw gyfnod allweddol ac sydd wedi ei asesu'n unol â'r asesiadau statudol yn y cyfnod allweddol hwnnw, rhaid i'r pennaeth, os gofynnir iddo wneud hynny gan riant y disgybl yn ysgrifenedig, drefnu bod lefelau cyrhaeddiad ac unrhyw ganlyniadau eraill y disgybl ym mhob targed cyrhaeddiad mewn unrhyw bwnc perthnasol ar gael i'r rhiant yn unol â'r rheoliad hwn.
(2) Rhaid i'r pennaeth gydymffurfio â chais o'r fath cyn pen pymtheng niwrnod ysgol ar ôl i'r pennaeth ei gael.
(3) Nid yw paragraff (1) yn gymwys os yw'r wybodaeth y cyfeiriwyd ati yn y paragraff hwnnw eisoes wedi bod ar gael i riant y disgybl yn unol â'r Rheoliadau hyn.
