5. Cofnod o’r holl arian neu bethau gwerthfawr eraill a adneuwyd gan yr unigolyn gyda’r gofalwr lleoli oedolion i’w cadw’n ddiogel neu a gafwyd ar ran yr unigolyn, a hwnnw’n gofnod y mae rhaid iddo gynnwys cofnod o—
(a)y dyddiad yr adneuwyd neu y cafwyd yr arian neu’r pethau gwerthfawr;
(b)y dyddiad y cafodd unrhyw arian neu bethau gwerthfawr—
(i)eu dychwelyd at yr unigolyn, neu
(ii)eu defnyddio, ar gais yr unigolyn, ar ei ran;
(c)pan fo’n gymwys, y diben y defnyddiwyd yr arian neu’r pethau gwerthfawr ato;
(d)y gydnabyddiaeth ysgrifenedig bod yr arian neu’r pethau gwerthfawr wedi eu dychwelyd.
