SCHEDULEE+W+SINFORMATION REQUIRED IN AN APPLICATION FOR APPROVAL AND AN APPLICATION FOR RENEWAL OF APPROVAL

1.  Name and address of the person submitting the application.E+W+S

(1) whether the meter is single or polyphase;

(2) whether it is an induction or electronic meter;

(3) whether it is a prepayment or credit meter; and
